Healed dissecting aneurysm in cystic medial necrosis of the aorta.

چکیده
DISSECTING aneurysm of the aorta has long been recognized as a disease entity, and is ordinarily regarded as a rapidly fatal condition. A certain proportion of dissecting aneurysms redissect into the lumen of the aorta, forming the so-called "double-barreled" aorta. Occasional cases of dissecting aneurysm have survived for a considerable period after the acute episode, many of them being of the "double-barreled" variety.'-7 Most of the reported prolonged survivals suffered from congestive heart failure and eventually died as a result of the aneurysm.2' 4, 7 Few cases have been reported in which the dissecting aneurysm was essentially an incidental autopsy finding of relative clinical insignificance.8 The following case report is presented as an instance of chronic, nondisabling dissecting aneurysm associated with idiopathic cystic medial necrosis.

CLINICOPATHOLOGIC CORRELATIONS Spontaneous Laceration of Ascending Aorta
Spontaneous laceration of the ascending aorta may result in (1) through-and-through laceration causing hemopericardium, (2) limited intramural dissection of blood (incomplete dissecting aneurysm), or (3) classical dissecting aneurysm. The latter is the most common manifestation. Background conditions include either extensive cystic medial necrosis (as may be seen in Marfan's syndrome) or hypert...
متن کاملCLINICOPATHOLOGIC CORRELATIONS Dissecting Aneurysm of Aorta Complicating Aortic Valvular Stenosis
Six cases of classical dissecting aneurysm of the aorta complicating aortic valvular disease in which aortic stenosis was dominant are described. In three of the cases the aortic complication occurred after the natural aortic valve had been replaced by a prosthetic valve. The longest postoperative period was four months. The factor common to all reported cases in our series was cystic medial ne...

متن کاملTurner's syndrome associated with bicuspid aortic stenosis and dissecting aortic aneurysm.
A case of Turner's syndrome is described associated with bicuspid aortic stenosis and fatal rupture of a thoracic dissecting aortic aneurysm. Histology of the aneurysm showed severe cystic medial necrosis. This association has not been previously described in the absence of coarctation. In view of the possibility of surgical repair, dissecting aortic aneurysm should be considered in all patient...
